  • Completed Supervisor/Worker Class “B” Application
  • Current Training Certificate
  • One (1) Current, Color Photo. Photo may be sent electronically.
  • Valid Government Issued Photo ID
  • Non-Refundable Fee – $20.00 (cash, company check or M.O. only)
    Credit Card Payments Accepted Online

NOTE: Class “B” certifications are issued on an annual basis.
